AFA Membership Form & Annual Subscription
To become a member of the AFA (ACT) please complete this Membership application form. This form can also be used to update contact details, including address and telephone numbers.

Part 1: Annual Membership
Annual membership includes subscription to the Adoption Australia magazine which is issued 4 times a year. If you are a New Member to AFA, there is a one off $20.00 joining fee.

Part 2: Annual Subscription to Adoption Australia
This magazine is issued 4 times a year. You can subscribe to Adoption Australia only with no membership.

Part 4: How to Pay
Once your Membership Form is complete, you can either:

• email the form (using the 'Submit' button above) and post your cheque or money order with a copy of the completed form to the address below, OR
  
• print out completed form and post it to us, together with your cheque or money order, to the address below, OR
  
• complete the internet banking transaction and email the transaction receipt together with completed form to the Membership Secretary.
