AFA Membership Form & Annual Subscription
Membership of the AFA (ACT) provides access to advice, support and resources for adoptive families.

To become a member of the AFA (ACT) please email the Membership Secretary for an application form. 

This form can also be used to:

• renew your membership
• update contact details, including address and telephone numbers.

The AFA will accept payment for annual membership and/or subscription for Adoption Australia via cheque, money order or internet banking.

Once your Membership Form is complete, you can either:

• complete the internet banking transaction and email the transaction receipt together with completed form to the Membership Secretary, OR
• email the form and post your cheque or money order with a copy of the completed form to the address below, OR
  
• print out completed form and post it to us, together with your cheque or money order, to the address below.
