AFA Membership Form & Annual Subscription
Membership of the AFA (ACT) provides access to advice, support and resources for adoptive families.

To become a member of the AFA (ACT) please complete the Membership Application Form. This form can also be used to update contact details, including address and telephone numbers.

The AFA will accept payment for annual membership and/or subscription for Adoption Australia via cheque, money order or internet banking.

Once your Membership Form is complete, you can either:

• complete the internet banking transaction and email the transaction receipt together with completed form to the Membership Secretary, OR
• email the form and post your cheque or money order with a copy of the completed form to the address below, OR
  
• print out completed form and post it to us, together with your cheque or money order, to the address below.
